I authorize the veterinarians and staff of Jog Lantana Animal Clinic to examine my pet and provide the services that I approve. I understand that recommendations, treatment options, and estimated costs will be discussed with me when appropriate.

I accept financial responsibility for all services I authorize. Payment is due when services are provided or when my pet is released.
